A friendly reader has provided me with a scan on a Xerox WorkCentre 7535: WH LFBC Scanned Xerox 7535 WC
The document’s creator and producer are identical: Xerox WorkCentre 7535
The document contains 17 layers. The jpg again contains the comment string YCbCr

The multiple monochrome bitmap layers show a more aggressively segmented document, however the sample document shows how a Xerox WorkCentre indeed creates multiple monochrome bitmap layers.
The images are also rotated 90 degrees clockwise, unlike the WH PDF which led me to the prediction that the staffer scanned the document upside down and used Preview to correct this.
The document shows that the text is JBIG2 encoded. When saving it by Printing to PDF, the JBIG2 is saved as FlateDecode and the resulting document looks just like the WH PDF.
Note how the jpg shows ‘colors’ behind the text, an artifact created by the JPEG compression.
The JPEG is 1664×1280 and the resolution appears to be twice that of the text layers (to be verified)
Obj 10 contains the rendering instructions. XIPLAYER0 contains the JPEG, XIPLAYER_CM1 the majority of the text (1403×1749). The instructions show that the JPEG is rendered at 150 DPI (1664/798.72*72) and the text at 300 DPI (1403/336.72*72)
